Welcome to the online application form for non-UT student program requests at the University of Tennessee Health Science Center (UTHSC). Completion and approval of this form is required before any program for non-UTHSC students can take place on the UTHSC's campus.

This online form will take some time to complete. You may return to this online form as often as you wish to enter information; however, once the form is submitted, changes can be made only by contacting Stacy Luckett or Stephanie Redd at (901) 448-5600.

If you are a UT employee, you may use your net ID and password to proceed to the application form. If not, you will be taken to a registration page where you will set up an account and specify a Username and Password. After registering, you may proceed to the application form.
