Welcome to the online application form for enrollment in Health Career Programs at the University of Tennessee Health Science Center (UTHSC). Completion of this application (including supplemental documentation) is required to apply for any program offered by Health Career Programs at UTHSC.

To learn more about the programs offered by the Health Career Programs, go to the Health Career Programs website. This online application will take some time to complete. You may return to it as often as you wish to enter information; however, once the application is submitted, changes can be made only by contacting the HCP office at 901-448-8772 or via email at hcp@uthsc.edu.

You will be taken to a registration page where you will set up an account and specify a Username and Password. After registering, you may proceed to the application form. You will not be able to create multiple logins, even if you wish to apply to multiple programs.
